PSM Related Services   


Hazard Assessment: used to determine how complex and comprehensive a facility's chemical risk management program must be. This component includes analyses of worst-case and alternative release scenarios. Up

Prevention Program: includes safety information and a review of process hazards, as well as operating procedures, training and maintenance. The program must also specific guidelines for incident investigations and compliance audits. Up

Management Program: requires official documentation of the complete chemical risk management and prevention program implemented by a facility. Up

Emergency Response Plan: is a plan of action designed for the organizations that may respond to an accident. It details what steps should be taken, by whom, to ensure the safety of personnel at risk, and to minimize the impact of the accident. Up

Employee Participation: The objective of the employee participation program is to involve and inform facility personnel in the development of the elements of the PSM program. PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in;

  • Development and training of the PSM Team
  • Presentation of the PSM responsibilities to PSM team and other personnel
  • Development of the documentation required by this element Up

Process Hazard Analysis: PSM Technical Services, Inc. will provide services to assist the facility in the conduct of a PHA. It is anticipated that the PHA will be conducted utilizing the "What-If / Checklist" technique. The purpose of a What-If / Checklist Analysis is to identify hazards, hazardous situations or specific accident events that could produce an undesirable consequence. An experienced group of facility personnel identify possible accident situations, their consequences and existing safeguards, then suggest alternatives for risk reduction. The method can involve examination of possible deviations from the design, construction, modification and operating intent. It requires a basic understanding of the process to be evaluated, along with the ability to mentally combine possible deviations from the design intent that could result in an accident. The What-If / Checklist Analysis will generate a list of questions and answers about the process. It may also result in a tabular listing of hazardous situations, their consequences, safeguards and possible options for risk reduction. PSM Technical Services, Inc. will:

  • Coordinate development of requisite chemical, technology and equipment information in preparation of the PHA
  • Prepare the PHA participants through a review of the PHA techniques, it's intent, it's conduct and it's expected outcome
  • Facilitate the conduct of the PHA, utilizing facility personnel, information and resources. The PHA will follow the Center for Chemical Process Safety Guidelines for the conduct of a "What-If / Checklist" PHA study. The study will be conducted and documented utilizing Primatech "PHA Works (v. 5.04)" software. It is anticipated that 2-4 sessions of 3-hour duration will be required and that the on-site portion of this study will be completed in one working site visit.
  • Develop documentation required for preparation, conduct and output of the PHA
  • Develop the documentation required by this element Up

Training: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:

  • Development of the training agenda and curriculum for the following PSM elements
    • Process Safety Information, including such topics as:
      • Covered Chemical properties
      • Safety and health hazards of the covered chemicals
      • An overview of the installed process
      • System components
      • Safety systems
      • Block flow and P& IDs prepared for the process
  • Operating procedures
  • Emergency procedures
  • Mechanical integrity
  • Contractor procedures
  • PSM program overview
  • Safe work practices and procedures including:
    • Confined space entry
    • Lock-out/Tag-out
    • Hot work permits
    • System entry (process breaks)
  • Development of the documentation required by this element Up

Pre-Startup Safety Review: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:

  • Development of the "Pre-startup Safety Review Checklist"
  • Development of the documentation required by this element Up

Hot Work Permits: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:

  • Review of the existing "Hot Work Permit Program" to identify all PSM requirements and to recommend modification as necessary
  • Reflection of the "How Work Permit Program" into the PSM Program Manual
  • Development of the documentation required by this element Up
Incident Investigation: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:
  • Review of the existing "Incident Investigation Procedure" to identify all PSM requirements and to recommend modification as necessary
  • Reflection of the "Incident Investigation Procedure" into the PSM Program Manual
  • Development of the documentation required by this element Up

Compliance Audit: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:

  • Development of the conceptual audit process to be utilized at the facility
  • Development of the documentation required by this element Up

Process Safety Information: PSM Technical Services Inc. will provide mentoring and coordination services to assist the facility in:

  • Identification, inventory and presentation of requisite process chemistry information
  • Identification, inventory and presentation of requisite process technology information
  • Identification, inventory and presentation of requisite process equipment information
  • Development of the documentation required by this element Up

Operating Procedures: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:

  • Identification of operating instructions required by the PSM standard
  • Development of operating instruction not present at the facility
  • Presentation of these operation instructions in a format approved by the facility and satisfying the criteria required by the standard
  • Development of the documentation required by this element Up

Contractor Control: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:

  • Development of "Plant Safety Rules for Contractors", reflecting the required information regarding potential hazards related to contractor work as well as applicable facility safety rules and emergency response provisions
  • Development of contractor safety performance assessment criteria
  • Development of the documentation required by this element Up

Mechanical Integrity: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:

  • Identification of equipment to be addressed under this element
  • Incorporate identified QA procedures that are applicable to the equipment-process. QA may be addressed within each of the procedures by including specific QA tasks whenever appropriate. However, additional QA procedures may be appropriate for activities that are not addressed by maintenance procedures, such as design engineering, equipment purchasing, major equipment fabrication, material receipt, spare parts management and field labor supervision. Management of Change: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:

  • Development of the Management of Change conceptual process
  • Development of the Management of Change check list to reflect the conceptual process
  • Development of the documentation required by this element Up

Emergency Planning and Response: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:

  • Review of the existing "Emergency Response Plan" to identify all PSM requirements and to recommend modification as necessary
  • Reflection of the "Emergency Response Plan" into the PSM Program Manual
  • Development of the documentation required by this element Up

Trade Secrets: PSM Technical Services, Inc. will provide mentoring and coordination services to assist the facility in:

  • Review of the process for identification of information to be treated as proprietary information
  • Development of the process of addressing requests for proprietary information as prescribed in the PSM standard
  • Development of the documentation required by this element Up
